Section 1 - Health Insurance and Supplemental Benefits

I want to enroll in health benefits for 2026

Eligibility: You are eligible for benefits if you work an average of 30 or more hours per week, or 130 hours per month. You may also enroll your eligible family members under certain plans you choose for yourself.  Please contact HR if you have questions about your eligibility.

 Eligible family members include:

  • Your legally married spouse.
  • Your biological children, stepchildren, adopted children, or children for whom you have legal custody (Age Restrictions May Apply)
  • Disabled children age 26 or older who meet certain criteria may continue on your health coverage.

What You Need to Do:

  • Determine if you are eligible for health benefits. If you are unsure, contact HR. 
  • Review the benefit guide.
  • Click the link below to complete the 2026 Benefit Plan Selection Form to ENROLL before the December 15th deadline.

What You Need to Do:

ACTION REQUIRED: Waive Health Benefits

  • If you DO NOT want health insurance, you must complete the waiver form. The process is quick and easy, taking less than 2 minutes to complete.
  • Click the link below to complete the 2026 Benefit Plan Selection Form for WAIVE health benefits before the December 15th Deadline

I want to change my current health benefits for 2026

What You Need to Do:

  • Review your benefits. 
  • Make Changes or waive coverage before the December 15th deadline. 
If you’re currently enrolled in health insurance and don’t make changes or waive coverage: 
  • Your current elections will stay the same. 
  • Deductions will continue to come out of your paycheck until the next Open Enrollment of qualifying life event.

Annual Acknowledgements 2026

Verify, and update if necessary, your Personal Information and Contact Information in your Paycor Profile for accuracy. After verifying, go to the Annual Acknowledgements 2026 Document in Paycor, complete your signature, and submit.

Instructions:

Step 1: Login to Paycor > Select Profile Summary from Home Screen > On the Left select “Personal” > “Personal Info” > “Contact” > then update “Emergency Contacts” as well.

Verify Personal Information and Contact Information. Click EDIT in the top right corner if you need to update any information. SAVE.

Step 2: Home > My Tasks > Annual Acknowledgements 2026

Read the document. Select the checkbox to confirm. Type your first and last name. SUBMIT.

OPT- INTO PAPERLESS W-2

In Paycor, every active employee can opt out of receiving a paper version of their Form W-2. Opting out of the paper version means you receive only an electronic copy of Form W-2.

  • You can print the electronic copy as many times as you want and it’s always stored in the Paycor application for future access and retrieval.
  • Form W-2 deadline for paperless:
    • The deadline has passed to receive your 2025 W-2 electronically, but you can choose paperless for 2026.
  • A confirmation email is sent to your email on record in Paycor when your consent waiver is electronically signed and saved.

Instructions:

  1. After you log into Paycor, on the left side under the Paycor logo, click Profile Summary.
  2. In the left menu, go to Pay & Taxes > Paperless Options. The Paperless Options section appears where you can enable paperless documents to receive your payroll forms electronically.

Remember, enabling paperless options for a document will halt the delivery of paper copies for the associated document.

All 1 year employees and employees making anniversaries of 5, 10, 15, 20, 25, and 30 years from the list, please come by the office to pick up your gift by 12/22/2025. Everyone else – be on the lookout for your gift in the mail.

Employee Assistance Program:
Counseling Sessions

OPTIONS is committed to supporting employees by providing meaningful assistance for personal challenges that may affect job performance. To achieve this, the Agency has contracted with two private counselors to offer confidential employee assistance and support.

Each employee is entitled to six (6) counseling sessions at no cost. Should additional sessions be needed, they must be requested and approved by the Chief Executive Officer or Human Resources Director.

To schedule a confidential appointment, employees should contact Human Resources for the counselors’ contact information. The process is managed with strict confidentiality to ensure employee anonymity.
